Application Forms
If you want to become a foster carer or adopt a dog, please complete an application form (see links below).
Click on the link and follow the instructions to download a blank form. Complete the form by opening the document in your computer’s editor and tabbing from field to field. Then print the document, sign the ”Declaration” and return a signed copy to CAWI. *
Note – on the Foster Carer Application form, you must sign both the “Foster Carer Guidelines” and the “Declaration”.

Remember to keep a copy for yourself
* CAWI requires your signature on a paper copy of the Adoption / Foster Carer Application Form before you take possession of a dog.
If you email us a copy too, we can speed up the process by reviewing your application, placing your name on the waiting list, and making a start on identifying a dog that matches your requirements and circumstances.
If you can’t download the form, please email us, phone us during business hours, or visit the op-shop for help.
